Person-Centered Planning
Person-Centered Planning is an ongoing problem-solving process to help people with disabilities plan for their future. A Person-Centered Plan is a tool in that problem-solving process that allows people with disabilities, their families, and their support professionals to learn more about preferences, goals, dreams, and challenges. Here, we’ve broken the plan into 12 accessible elements and included descriptions and tools to help you along the way.

The Person Centered Plan Template is the first step in creating a Person Centered Plan for the individual(s) you support. Use each tool to help you fill out the sections of this template and create a complete picture of the unique person you’re supporting.

Who’s got your back?

What are some things you dislike? What would make for a bad day? Tools in this section will help you document the items, issues, situations that are non-negotiable for you.
Your Preferences & Choices
What choices do you make that are important to you? What are some of your preferences related to how people interact with you? Tools in this section will help you identify and document your preferences in communication, activities and environments.

What is your story? Tools in this section help identify your family history, special events in your life, stories about you, defining moments and people who are special to you.
