Transition Age Youth

Transition Age Youth Programs

Our Transition Age Youth programs are designed to support youth ages 14 to 25 with a disability diagnosis, as well as their families. Through these programs, we help connect families to vital resources, including Transition Aged Youth benefits applications assistance through Benefits Enrollment Center (BEC), mental health support through Project LIVE, career and job exploration advocacy through our Ready@21 program. Our mission is to empower youth with intellectual and developmental disabilities to thrive and live their most independent lives.

If you have a graduating youth, and are interested in adult services, please contact
